The following portal will enable you to securely submit a form to the clinic. Please note that all submissions are reviewed by our administrative staff before they are uploaded to your chart.

DO NOT submit a photo unless you have booked an appointment to speak to your physician, or unless it has been requested by your physician directly. 

DO NOT submit any photos of a sensitive nature.
